Severe loss of posterior mandibular bone can leave limited height above the inferior alveolar nerve for implant placement. Nerve lateralization is an advanced surgical option considered in selected patients when an individualized assessment supports this approach. Planning focuses on the nerve canal, the remaining bone and the intended fixed restoration.

The procedure involves controlled access to the lateral mandible and careful mobilization of the nerve to create the required implant pathway. The surgical strategy is coordinated with the restorative plan, with particular attention to tissue handling and the management of the cortical window.
